What are Chronic Conditions Special Needs Plans? 

C-SNPs are a unique Medicare Advantage plan (aka Medicare Part C) created for people living with chronic illnesses. These plans go beyond the standard benefits of traditional Medicare Advantage plans by offering tailored services and resources to address the specific health needs seen in patients with specific chronic conditions. Think of them as a more personalized and focused approach to healthcare — designed with the condition — and the individual — in mind. 

Because nearly two-thirds of Medicare enrollees are juggling multiple chronic conditions, careful coordination between healthcare providers, primary care doctors, mental health professionals, and other health specialists is required.  

Beyond that, many rely on a network of inpatient and outpatient facilities, often including highly specialized diagnostic and therapeutic services. C-SNPs combine all these elements to create a seamless and well-coordinated approach to care that helps patients — and their caregivers, if appropriate — manage their health more effectively. 

For example, if you have emphysema, a C-SNP might include benefits like access to respiratory therapists who can help improve lung function, coverage for inhalers or oxygen therapy, and pulmonary rehabilitation programs to help you manage symptoms and improve your quality of life. 

Similarly, for multiple sclerosis, a C-SNP could provide access to neurologists, physical therapy programs to improve mobility and strength, and medications to help manage symptoms and slow disease progression.

Who is eligible for a C-SNP? 

Chronic Conditions Special Needs Plans are an excellent option for those requiring ongoing, specialized care to stay healthy and active.  

C-SNP eligibility rests on two key requirements: 

  1. You must be enrolled in both Medicare Part A (hospital insurance) and Medicare Part B (medical insurance). Only with these basic Medicare coverages in place will you be able to enroll in a C-SNP. 
  2. Then, of course, you must have a chronic illness that qualifies for a C-SNP. If you’re not sure if your condition qualifies, your healthcare provider can help you confirm eligibility. 

What are chronic conditions? 

Understanding chronic conditions and their similarities can help you or a loved one take the first steps toward better management and care. These are serious illnesses, but with proper management, many people living with these conditions find ways to maintain a good quality of life. 

The Centers for Medicare & Medicaid Services (CMS) lists 15 disease states that can be covered under a Chronic Conditions Special Needs insurance plan. These include: 

  1. Autoimmune disorders, including arthritis and rheumatoid arthritis 
  2. Cancer 
  3. Cardiovascular disorders 
  4. Chronic alcohol and drug dependence 
  5. Chronic lung disorders, including asthma, chronic bronchitis, and emphysema 
  6. Dementia 
  7. Diabetes 
  8. End-stage liver disease 
  9. End-stage renal disease requiring dialysis 
  10. Hemophilia, myelodysplasia, sickle-cell disease, and other hematologic disorders 
  11. Heart failure 
  12. HIV/AIDS 
  13. Mental health conditions, including bipolar disorder, major depression, paranoia, schizophrenia, and schizoaffective disorder 
  14. Neurological disorders, including epilepsy, paralysis, ALS, Huntington’s disease, multiple sclerosis, and Parkinson’s 
  15. Stroke 

What do the 15 chronic conditions have in common? 

  • They usually progress slowly. Chronic conditions often develop over time rather than appearing suddenly. 
  • They’re typically not contagious. Unlike infectious diseases, chronic conditions can’t be passed from person to person. HIV/AIDS is the exception. 
  • They are life changing. Chronic conditions impact daily life, affecting activities, day-to-day functioning, enjoyment of hobbies, family gatherings, and overall well-being. 
  • They require ongoing management. Treating chronic conditions often involves a caregiver’s help and the use of medications and durable medical equipment (DME) like blood glucose monitors, insulin pumps, wheelchairs and scooters, nebulizers, prosthetics, and orthotics. 

How do C-SNPs work, and what do they cover?
